The Ages and Stages Questionnaire is an online screening tool used to monitor your child’s development and growth from birth through age five. You will be instructed to complete this questionnaire prior to your child’s scheduled well-child exam. Please do not complete this questionnaire unless instructed to do so by our staff.

This screening tool is age-specific and your child must be within a specific age range in order for the questionnaire results to be accurate. The results are not available online, and must be interpreted by your child’s provider.
